Tips for searching by name
- Use the legal name. NPPES stores the provider’s legal first and last name, not nicknames or professional aliases.
- Married or maiden name. If a provider recently changed their name, their NPPES record may still list the former name — try both.
- Organizations. Search a distinctive part of the business name in the organization field; the match looks anywhere within the registered name.

Frequently asked questions
How do I look up an NPI by name?
Enter the provider’s first and last name (or an organization name) and, optionally, a state. Results link to a profile showing the NPI number, taxonomy, and practice address.
The provider changed their name — which name do I search?
Search the name registered with NPPES. After a marriage or legal name change, the NPPES record may still show the maiden or former name, so try both if the first search comes up empty.
How do I find an organization’s NPI?
Use the organization name field. Type a distinctive part of the legal business name — the search matches organization names that contain your text.
Why are there several providers with the same name?
Common names return multiple records. Use the state filter and compare specialty and city on each result to identify the right provider before opening the profile.
